How to Make a Survey

Click the link and follow the steps below:

Google Docs Sign In1-At the top left click CREATE NEW and go to FORM

2-Title your form with your GROUP NAME and NUMBER: EX. Purple-A3

3-Type your question in QUESTION TITLE

4- Under QUESTION TYPE, choose MULTIPLE CHOICE

5-Type your options in the spaces below: Make sure you have 3-5 options for each question

6- Click MAKE THIS A REQUIRED QUESTION

7-Repeat for all SEVEN questions

8- For SAMPLE QUESTION 8, click the TRASH CAN icon to delete it

9-Click SAVE in the top right

10-Go to MORE ACTIONS and click EMBED

11-Highlight and copy the link

12-Go to your Wiki page, click SURVEY and follow those directions to post your survey!
